We are responsible for the confidentiality of these records.

I, the client agree to my records being held under the above terms and I certify that the information I have provided is correct to the best of my knowledge.

I confirm that I have discussed all my options with the relevant staff and any concerns have been addressed with the treating therapist.

I give permission for the agreed treatments to be carried out on my child and that Petite Pamper staff are not liable for any adverse events that may occur as a result of these treatments.

I have given an accurate account of the questions asked above including all known allergies or prescription drugs or products currently being used by my child.

I understand that the staff at Petite Pamper will take every precaution to minimise or eliminate possible reactions however they can still occur as each individual is different and can react in different ways.

In the event that I may have additional questions or concerns I will raise this with the staff at Petite Pamper immediately.

I understand that no results can be guaranteed and that although we strive to give you the best experience possible we at Petite Pamper will not be liable or held responsible if your treatment/party does not meet your expectations.

I understand that Petite Pamper operates a NO REFUND policy.

I understand that party packages can sometimes be bespoke for each client so party packages and prices may differ between individuals.

I understand that for the safety and wellbeing of staff and children we have safety cameras installed in our rooms in case of any adversities. These are only looked at by Petite Pamper staff if needed to clarify any specific situations.
